THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED, AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

1Our commitment to your privacy

Kleopatrea LLC, d/b/a Kleopatra Health ("Kleopatra Health," "we," "us"), is required by law to maintain the privacy of your Protected Health Information ("PHI"), provide you with this Notice describing our legal duties and privacy practices, and abide by the terms of this Notice as required by the Health Insurance Portability and Accountability Act (HIPAA).

2How we may use and disclose your PHI

Treatment

We may use and disclose your PHI to provide, coordinate, or manage your care — for example, sharing information between a provider and a pharmacy to fill a prescription.

Payment

We may use and disclose your PHI to bill and collect payment for services, including verifying insurance coverage or eligibility.

Healthcare operations

We may use and disclose your PHI for internal operations such as quality assessment, provider training, and business planning.

As required or permitted by law

To public health authorities for reporting purposes
To report suspected abuse, neglect, or domestic violence, where required
In response to a court order, subpoena, or legal process
To avert a serious threat to health or safety
To coroners, medical examiners, or as otherwise required by law

Any other use or disclosure of your PHI not described in this Notice will only be made with your written authorization, which you may revoke at any time.

3Your rights regarding your PHI

Right to access — request a copy of your medical and billing records.
Right to amend — request a correction to your PHI if you believe it is incomplete or inaccurate.
Right to an accounting of disclosures — request a list of certain disclosures we've made of your PHI.
Right to request restrictions — ask us to limit how we use or disclose your PHI for treatment, payment, or operations.
Right to request confidential communications — ask us to contact you by a specific method or at a specific location.
Right to a paper copy — request a paper copy of this Notice at any time, even if you agreed to receive it electronically.
Right to file a complaint — if you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services, without retaliation.

4Our responsibilities

We are required to maintain the privacy and security of your PHI, notify you promptly if a breach occurs that may have compromised your information, and follow the terms of this Notice currently in effect.

5Changes to this notice

We reserve the right to change this Notice and to make the revised Notice effective for PHI we already have as well as any information we receive in the future. The current Notice will always be available on this page and, upon request, at our office.
